Requirements:

  • Up-to-date immunization records
  • Social Security numbers (sponsor and children)
  • Current health assessment must be filed within 30 days
  • Copy of current physical (less than one year old) is required to participate in sports programs
  • Proof of income (sponsor and spouse)  
  • Two emergency contacts other than parents (must be local)
  • If a child has asthma, they will need a Respiratory Medical Action plan signed by a physician
  • If a child has seizures, they will need a Seizure Medical Action plan signed by a physician
  • If a child has SEVERE allergies requiring Benadryl and/or an EPI Pen, they will need a Severe Allergy Medical Action Plan and a Diet Statement (if it is food allergy related) signed by a physician
  • If your child has diabetes, they will require a Diabetic Medical Action Plan signed by a physician
  • Registration is FREE and is valid for one year
  • Family Care Plan for dual and single military must be filed within 30 days

Below lists some of the Parent Central Services forms you will need. They have been provided for your convenience - Please print them, fill them out, and bring them to Parent Central Services (Building 1518). They require the Adobe reader to print.
 
For the Waitlist Form, Click Here. Please fill it out and print it for delivery to the Parent Central Services office. If you would like to fax it, please use fax number (719)524-0194, or DSN 883-0194. If the sponsor and/or spouse is military, please include the military rank in the name box on the form.  Please note - If you have Adobe Acrobat 9 Pro, this form is now available to be emailed directly to our Parent Central Services office.
